About the Role:

The Nurse Practitioner, in collaboration with the Center Physician, provides a wide range of services, such as conducting physical examinations, assessing, and ordering tests and interpreting the results, conducting on-site diagnostics, counseling patients on preventive care, and preventive and health maintenance services. The Nurse Practitioner works under the medical supervision of the Center Physician. Provides exceptional 5-Star patient experience by spending more time with patients (lower patient volumes), building long-standing relationships through a personalized, concierge approach to age management medicine care for patients’ healthcare.

What you will be doing:

  • Perform evidence-based practice screenings including point of care testing (as appropriate)
  • Identify diagnoses to be used in care management and active medical management in furtherance of treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and judgement and findings of your assessment
  • Conduct physical examinations, initial and follow-up consultations including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
  • ·Review and interpret comprehensive patient diagnostics and make personalized program recommendations based on results
  • Prescribe medications and nutritional supplementation as indicated and in accordance with Cenegenics protocols
  • Order labs, additional diagnostics as required and in accordance with Cenegenics protocols
  • Check in with patients about test results and prognoses
  • Contact and consult patients through various forms of communication, i.e. Email, text, phone and in person
  • Maintains confidentiality of all patients and EMR documentation via Salesforce EMR
  • Advises patients regarding prescriptions and treatment plans
  • Follows all federal, state, and nursing guidelines, regulations, and standard

Requirements:

  • Master’s degree in nursing. Doctor of Nursing Practice (DNP) degree preferred. Active ANCC or AANP national certification, academic degree, or completions of a Nurse Practitioner.
  • An unrestricted, current Nurse Practitioner license in California, Ohio or Texas.
  • Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
  • The Nurse Practitioner must have and maintain board certification by the American Nurse Credentialing Center (AANC)
  • 5+ years of nursing experience
  • 3+ years’ experience in adult family practice or preventive medicine
  • Valid DEA license
  • Knowledge of HIPAA, OSHA and for California candidates: The California Confidentiality of Medical Information Act (CMIA) regulations
  • Able to operate as part of a team, alongside physicians and other clinical staff
  • Proficient in EHR medical charting system and computer-based data management programs and information systems
  • Adhere to all federal, state and local regulations as well as all client requirements
  • May be requested to obtain additional licensure in other geographic areas
